Hospice Simcoe-Seasons Centre Residence

The 10 bed residence at Hospice Simcoe-Seasons Centre gives people who are living with a palliative diagnosis the opportunity to live with dignity and comfort in a place that feels like home. We are able to provide 24 hour nursing care including pain and symptom management, social, psychological, spiritual and practical support. We also support family members with their bereavement needs. Our average length of stay is about 11 days, however it can be as brief as a few hours or as long as a few months.

The staff and volunteers welcome you and your family to Hospice Simcoe-Seasons Centre. We encourage you to make our residence your ‘home away from home’. The staff and volunteers will endeavour to provide you and your family with compassion, support and care during this time. We are committed to creating an inclusive environment where all traditions, cultures, abilities and lifestyles are celebrated and honoured. Privacy and confidentiality are respected and upheld by all staff and volunteers. We are committed to person-centred care which respects and values the individual’s right to dignity and self-determination when it comes to their end of life experience.

What You Need to Know

A referral request must go to the Ontario Health atHome where they will complete an assessment and referral prior to admission to Hospice Simcoe. The request can come from a physician, client or substitute decision maker for the client. We encourage people to include hospice care in end of life planning as part of advance care planning. Admission to the residence at Hospice Simcoe-Seasons Centre is based on a triage assessment tool that identifies the person most needing an admission.

Hospice Simcoe-Seasons Centre does not charge for our programs/ services. We rely on donations, including in memoriam gifts, general donations and third party events for 50% of our operating costs.

The average length of stay that someone stays at our residence is 9 days. However; it can be as brief as a few hours up to a few months. Care is a client centred approach meeting the needs of clients and families.

The palliative care program is for individuals and families who can benefit from an alternate choice other than their home or hospital.  The criteria for admission to the residential home includes having a diagnosis of a life threatening illness with a prognosis of less than 3 months. Home and Community Care (HCC) will assess eligibility for a referral, and Hospice Simcoe-Seasons Centre will triage clients when they are ready for admission.

Hospice palliative care is aimed at improving the quality of life for persons who are living with or dying from advanced illness. The goal of palliative care is to provide comfort and dignity for the person living with the illness as well as the best quality of life for both the person and family when a cure is no longer. An important objective of palliative care is to relieve pain, suffering and other symptoms. Palliative care meets not only the physical needs, but also the psychological, social, cultural, emotional and spiritual needs of each person and family. Hospice palliative care helps people to live out remaining time with comfort and dignity in a homelike environment while receiving exemplary, compassionate 24/7 nursing care.

Spiritual Care

Spiritual care is about supporting you and your loved ones with those aspects of life that matter most. Spiritual care can be offered in a variety of ways: talking about meaning in life, family, faith, beliefs; receiving emotional, psycho-social, and grief support; engaging in music, art, legacy projects, nature; and through sacred rituals and support of faith traditions. We work as a team of staff and volunteers to provide such support in a caring, creative, and compassionate environment. We also work with community partners and have a list of providers from different faith traditions. Please speak to our Spiritual Care Coordinator or our clinical team and volunteers for more information.
