Hospice Simcoe is facilitating a weekly chat forum for health care providers throughout the month of May. Having the skills and knowledge to engage in Advance Care Planning (ACP), Goals of Care (GoC) and Person-Centred Decision-Making (PCDM) conversations with patients and family members is more important now than ever. Sometimes we feel overwhelmed by all the information that is out there. We will be sharing information and resources, and having great discussion around Advance Care Planning. We would love for you to join us to to learn more about this important topic via a Q&A.
